Some of these forms change from year to year so please check the version/dates and school listed on the form to ensure you have the most up-to-date form for your student's needs.
Acceptable Use Policy form - This form is a required sign-off prior to using the district's technology resources.
Athletic Code of Conduct and Handbook - Form A - This form is a required sign-off prior to participation in athletics.
Athletic Emergency Contact - Form B - This is an emergency medical information form that must be filled out and signed off prior to participation in athletics.
Athletic Physical Examination - Form C - This is the physical medical information form that must be filled out and signed off by a physician prior to participation in athletics.
Connecticut IEP Form This form is part of the Special Education process and has legal requirements associated with its completion. It can only be completed in association with a sanctioned special education intervention and cannot be filled out unilaterally by one party. It is provided here for informational purposes only.
Field Trip Permission Health Form - This is an emergency medical information form that must be filled out and signed off prior to participation in off-campus field trips.
HUSKY Application - This form is for qualifying (income) families who are interested in Connecticut's subsidized children's health insurance program.
Free and Reduced Lunch Application - This form is for qualifying (income) families who are interested in Connecticut's subsidized breakfast and lunch programs.
Lunch and Meal Information - This is the annual letter (06-07) that is sent home to parents explaining meal prices and options across the district.
OTC Pain Reliever Permission - This form must be filled out by parents if they want the nurse to be able to dispense over-the-counter pain relievers (Advil or Tylenol) to their son or daughter.
Student Information - This is the form OHS uses for emergency contacts
